ABSTRACT

<p><b>OBJECTIVE</b>To observe the difference in the analgesic effect on primary dysmenorrheal between acupuncture and sham acupuncture at Sanyinjiao (SP 6) during menstrual pain and evaluate the impact of psychological effect on acupuncture analgesia.</p><p><b>METHODS</b>Sixty subjects were randomized into an acupuncture group and a sham acupuncture group, 30 cases in each one. The conventional acupuncture and sham acupuncture were applied to Sanyinjiao (SP 6) on bilateral sides when menstrual pain began to attack and needles were retained for 30 min each time. Three menstrual cycles were required. The visual analogue scale (VAS) was adopted to determine the scores before and 0. 5 h, 1 h, 3 h, 6 h and 12 h after acupuncture during menstrual pain in each cycle separately.</p><p><b>RESULTS</b>In the acupuncture group, VAS score at each time point after acupuncture was reduced as compared with that at the previous one during menstrual pain in each menstrual cycle, indicating the significant difference (all P<0. 05). In the sham acupuncture group, the scores in 6 h and 12 h of acupuncture were reduced as compared with the previous one, indicating the significant difference (all P<0. 05). After acupuncture, VAS score at each time point in the acupuncture group was lower than that in the sham acupuncture group (all P<0. 05).</p><p><b>CONCLUSION</b>The conventional acupuncture at Sanyinjiao (SP 6) achieves the significant analgesic effect on primary dysmenorrheal. The psychological placebo effect of sham acupuncture has no obvious impact on acupuncture analgesia.</p>

ABSTRACT

@#BACKGROUND: Acute pancreatitis is a serious complication during pregnancy, however the incidence of hyperlipidemia induced by pancreatitis is lower. METHODS: We treated a pregnant woman with hypertriglyceridemia-associated acute gestational pancreatitis who simultaneously developed hypoxemic acute respiratory failure (ARF). RESULTS: The woman was successfully treated through noninvasive positive pressure ventilation (NPPV), emergent caesarean delivery, drainage of chylous ascites, and peritoneal lavage. CONCLUSION: The signs and symptoms of ARF were greatly improved in this patient after NPPV and conventional therapies. Early NPPV may be related to good prognosis of the disease.

ABSTRACT

Objective To evaluate the value of immunofecal occult blood test (IFOBT) as a prognostic indicator in CKD patients with colorectal impairment.Methods A total of 176CKD patients and 180 healthy adults as control were enrolled.Serum biochemistry was measured at baseline and gastrointestinal bleeding was determined by IFOBT.All the CKD patients were followed up for 4.5 years.Renal replacement therapy or death was defined as end-point event.The Logistic regression analysis was used for risk factors.Kaplan-Meier analysis and COX regression model were used for survival analysis.Results The positive rate of IFOBT in CKD patients was significantly higher than healthy control (17% vs 5.3%,χ2=13.236,P<0.01).When comparing with IFOBT negitive patients,IFOBT positive patients were older [(62.030±15.544) years old vs (48.660±19.018)years old,P<0.01],had higher ESR [(71.800±31.657) mu/h vs (57.210±32.712) mm/h,P<0.05],C-reactive protein [6.230 (3.000~14.148) mg/L vs 3.000 (3.000~6.833)mg/L,P<0.05],serum creatinine [419.100 (103.200~546.625) μmol/L vs 175.100 (68.150~462.950) μmol/L,P<0.05],and had lower hemoglobin level [(97.970±20.590) g/L vs (107.170±27.988) g/L,P<0.05] and eGFR [11.400 (8.671~53.544) ml·min1·(1.73 m2)1 vs 35.274(10.961~82.145) ml·min-1·(1.73 m2)-1,P<0.01].There was a negative correlation between IFOBT value and eGFR in CKD patients (r=-0.20,P<0.01).Positive correlations of IFOBT value with age (r=0.175,P<0.05) and serum creatinine (r=0.171,P<0.05) were found.Logistic regression and COX regression analysis showed that IFOBT value,eGFR and ESR were important factors that influenced the prognosis of CKD patients.Kaplan-Meier analysis revealed that IFOBT value >100μg/L predicted progression of renal function.Conclusions The prevalence of gastrointestinal bleeding disorder is high in patients with CKD.Value of IFOBT independently predicts decline in renal function of CKD patients.

ABSTRACT

Serum pharmacochemistry of traditional Chinese medicine (TCM) is a direct and effective method for determining efficacious substance foundation of TCM. However, the complexity of chemical constituents of TCM and the interaction among ingredients in the in vitro process make the analysis on in vitro ingredients of TCM arduous. Liquid chromatography coupled with tandem mass spectrometry (LC-MS/MS) has become the cornerstone in detection and characterization of in vivo ingredients of TCM because of its sensitivity and ability to analyze complex mixtures. However, due to significant interference from endogenous species, detection and identification of the constituents of TCM in the biological matrices are often difficult. There is a crying need for introducing specialized ingredient identification techniques to avoid artificial omission of in vivo ingredients of TCM. On the basis of the analysis on transitional ingredients in rat blood, this essay introduces the application of such pattern recognition methods as mass defect filter, Metabolynx software and principal component analysis, partial least squared discriminant analysis and orthogonal partial least squared discriminant analysis in identifying in vivo ingredients of TCM.

ABSTRACT

Platelets carry over 20 growth factors, which all have been shown to improve wound healing, particularly recalcitrant wound healing. The aim of this study was to investigate the healing effect of lyophilized platelets on the chronic wounds through establishing diabetic rat chronic wound model. Healthy male SD rats were intraperitoneally injected with streptozotocin (STZ) solution at the dose of 65 mg/kg. The blood glucose and weights were observed every week. The re-epithelialization rates of normal control group (NDR), diabetic group (DR) and diabetic treatment group (TLP) was analysed. Two full thickness skin wounds were incised in the back of the rats. The re-epithelialization rates were observed at 1, 3, 5, 7 and 12 days. The results showed that after induced by streptozotocin for 72 hours, the blood glucose of the DR group was higher than 16.7 mmol/L. 1 week after induced by STZ, the weight of the DR group was significant lighter than that of the NDR group (p < 0.05). The re-epithelialization rate of DR group were lower than that of NDR. After 12 day treatment, the re-epithelialization rates of NDR and TLP groups were 88.1% and 81.8%, which were significantly higher than that of DR group (62.8%). It is concluded that diabetic rat model established by the intraperitoneal injection of streptozotocin can be used as a better diabetic chronic wound model. And the lyophilized platelets have healing effect on diabetic chronic wounds.

ABSTRACT

<p><b>OBJECTIVE</b>To investigate the therapeutic effect, long term survival and side effect on NSCLC patients treated with nadaplatin combined with paclitaxol and cisplatin combined with paclitaxol.</p><p><b>METHODS</b>NSCLC patients with stage IIIB or IV were randomized into two groups in this prospective clinical study. TN group: nadaplatin 30 mg/m2 dl-3, paclitaxol 175 mg/m2 dl, repeated every 4 weeks. TP group: DDP 30 mg/m2 dl-3, paclitaxol 175 mg/m2 dl, repeated every 4 weeks.</p><p><b>RESULTS</b>Sixty patients were enrolled and 57 were evaluable with 30 in TN group and 27 in TP group. The overall response rate were 43.3% vs. 48.1% (P = 0.716), and the disease control rate were 86.7% vs. 88.8% in TN and TP group (P = 0.799), respectively. The median survival time was 14.3 vs. 13.0 months, and the 1- and 2-year survival rate was 62.5% vs. 59.1%, 0% vs. 5.8% in TN and TP group (P = 0.839), respectively. The rates of neutropenia and thrombocytopenia were similar in TN and TP groups whereas more patients in TP group than in TN group suffered from anemia (38.5% vs. 17.5%, P = 0.001), nausea and vomiting (82.6% vs. 35.6%, P = 0.000), fatigue (35.9% vs. 14.1%, P = 0.000) and peripheral neurotoxicity (50.0% vs. 21.9%, calculated by case, P = 0.023).</p><p><b>CONCLUSION</b>Nadaplatin combined with paclitaxol is an effective treatment regimen for NSCLC patients. When compared with similar regimen with cisplatin, the response rate and survival were similar; however, nadaplatin regimen shows some superiority as regards some treatment side effect.</p>
